Investigating Incidents and Accidents course provides learners to implement effective investigation techniques aimed towards enhancing workplace safety and preventing future incidents. This training aims to provide professionals with the skills and knowledge required for carrying out comprehensive incident and accident investigations. These skills include root cause analysis, evidence collection, and analysis, witness interviews, and documenting findings. This course covers incident reporting procedures, accident causation models, legal and regulatory requirements and corrective action planning, among others. Proactive prevention strategies, compliance with safety standards, risk analysis, and safety standard OSHA, ISO 45001, and HSE guidelines integration receive particular focus. At the conclusion of the course, participants will be able to perform systematic investigations and identify factors that contributed to incidents as well as develop solutions that can enable improvement of organizational safety performance.

This course is designed to help learners properly examine incidents and accidents in the workplace. They will be able to put into action processes for determining actual causes and corrective measures that will stop such events from repeating themselves.

Preparation for a More Effective Investigation

  • Understand the importance of scene security and evidence collection after an incident has occurred.
  • Understand the functions of the investigation team and ways to remain impartial.

Analyzing a Scenario's Root Cause

  • Practice “5 Whys” techniques, Fault Tree Analysis and Fish Bone Chart method.
  • Outline a set of immediate, underlying and root causes of accidents and unsafe conditions.

Filling in Details and Report Writing

  • Create legally compliant organizational investigation reports.
  • Accompany witness statement documents with site photographs, timelines and corrective measures.

Developing Corrective and Preventive Actions Plans

  • Make systematic realistic action plans that investigations findings point out.
  • Oversee how effective control measures are for long-term safety improvements.

Training will be conducted via Microsoft Team Meeting, online. Meeting invites will be shared on the day before the first day of training.
